On this page you will find all the information you need to know about how we store and share your confidential information when you access our service.

If you have any questions or concerns about how your information is used, stored or shared you can call using on 0114 226 4380 and ask to speak to a member of our team.

It is really important to us that you are happy with the arrangements we have made for your care, so please do get in touch with us if you have any concerns.

What kind of information do we store?

The Sheffield Talking Therapies service mainly offers treatment from within each GP practice across Sheffield and therefore records information using the GP system.

Records are all stored electronically each time we see or speak to you on our confidential Patient Management System. The information we store could include a brief summary about your background taken during an assessment or treatment session, results of questionnaires about your mood that are specific to your treatment with the Sheffield Talking Therapies service, details of the treatment provided and any correspondence related to this.

It is important that you tell us within one week if you change your details, telephone number or address because we will continue to use the address and telephone numbers you have given us until you tell us they have changed.

What is our Patient Management System used for?

This is a secure and confidential NHS database where the service records information about your treatment. This system is used by staff to plan and monitor the quality of your treatment.

How you can access your records

We are happy to provide you with a copy of your records. If you would like to access your records please complete a request form at www.shsc.nhs.uk/accessrecords

When would we share information with other services?

You have control over who is involved in your care and this service observes strict NHS standards of confidentiality. The only time we will inform others without your permission is if we are concerned for your immediate safety, for the safety of someone else or if a British Court orders the release of your records.

We will share information with your GP about your care. This is usual in the NHS as your GP is the main person who organises your care. If you do not want us to keep your GP informed please make sure you call us to discuss this.

How does the service use questionnaires and other information to improve my care?

After you have completed questionnaires the information is added to our confidential Patient Management System. We use the results with you during treatment session to guide therapy. You can discuss this with your therapist is you would like more information.
