If you would like to self-refer there are a few different ways you can get in touch with us.

If you have a question or you would prefer to speak to someone on the phone you can give us a call on 0114 226 4380.

We know how difficult that first call can be and they will support you in any way they can to help explain the process and guide you through it. If you have any questions, they will be happy to help you.

The team is available Monday to Friday 8.30am to 5pm.

Feedback

Your feedback is important to help us continue to improve. If you have any feedback, good or bad, we want to know. 

Some of the methods we use are anonymous, or you will not receive a direct response.

If you have any concerns during your treatment, we encourage you to speak to your therapist or you can call 0114 2264380 and ask to speak to the duty manager. This way we can hopefully respond in a timely way to resolve the issue. 

Below are the main ways you can share your feedback:

  • These are set Talking Therapies questionnaires that ask you about your experience of therapy and the treatment you recieved.
  • They are anonymous, we are unable to reply.
  • We send these via text message to all patients. If you do not receive one, you can speak to your therapist or call 0114 2264380 to request the link (you must be in treatment with us).  
  • You will receive an Assessment PEQ in the week after your assessment.
  • You will receive a Treatment PEQ when you have finished treatment - two or more treatments, either planned finish or if you can no longer attend. 
  • We review the feedback monthly and look for any themes, positives as well as improvements.  
  • Care Opinion is an independent non-profit feedback service. It is used by a range of different NHS and voluntary organisations.
  • Patients or carers can share their story and experience - upto 1000 words, from the last 3 years.
  • Your story is posted anonymously online and we as a service can post a response. Stories and responses are visible to anyone who visits the website.

  • We would aim to provide an initial response within 72 hours. We will then provide any follow up as soon as possible. 
  • We are always looking for ways that our patients can have a say and be involved in making decisions about the service.
  • We have previously run engagement events looking at different areas for improvement,.
  • Sometimes we send anonymous questionnaires to current or previous patients.

How to make a complaint
