How to Self Refer

We encourage self-referral into our service. 

This allows you to manage your appointments and 

be ready to take part in your treatment.

Please read the following information to help you complete the referral form.

If you have any questions, or get stuck on the referral form please call admin team on 0114 2264380. We are here to help you. 

Once you have read through the information, please use the 'book now' button at the bottom of this page to complete your referral form.

  • I am 18+ years
  • I live in Sheffield and I am registered with a Sheffield GP
  • I am feeling down, worried or stressed
  • I am ready to make some changes
  • I can prioritise and manage regular appointments

  • I have/had a diagnosis/symptoms of Personality Disorder, Psychosis, Bipolar Disorder
  • I need urgent help and cannot keep myself safe
  • I am receiving support from another mental health service or therapy
  • I want to work on childhood abuse or have experienced many traumatic events
  • I have significant problems with alcohol or drug misuse

We value your privacy. We keep all information safe, following the General Data Protection Regulation (GDPR).

We store all patient information on a dedicated patient management system. It keeps your data safe and secure, following the Data Protection Act 2018. This is only accessed and used by Talking Therapies staff to plan and track your treatment. We will share information with your GP about your care. This is usual practice in the NHS as your GP is the main person you organises your care. We may also want to share information with other professionals in your care. This helps ensure you receive the correct treatment at the right time. We will ask your permission to share any information. 

We may need to share information with other professionals without your permission if we are concerned about your safety, the safety of another person, or if you inform us that you have committed or are about to commit a serious crime. Should this be the case, we will always try to discuss this with you first and involve you in the process. From more information read our privacy page on our website. 

Contact details 

By giving us your mobile number and email address, you agree to receive information from us about your appointments. We will not use your contact details for marketing or give them to any third parties.

Your telephone number will need to be able to receive withheld numbers. If you have any concerns about this, please call us on 0114 2264380.

Prefer not to say

We are aware there are a lot of questions on the form. Some of these may seem personal, like marital status, religion, sexual orientation, and gender. We gather this information so we can ensure our service is accessible to all. We understand you may not want to answer these questions, so please choose 'prefer not to say' from the drop down options.

Language

If you struggle with written English, please ask a friend or relative, support worker or health professional to help you complete the online referral form. Unfortunately the referral form is not translatable. All our pages on the website can be translated or easy read by clicking the 'make this page accessible' on the right hand side of the page. You can download our service booklet in the following languages; Arabic, Farsi Persian, Polish, Slovak and Urdu. We have leaflets available in Simple Chinese and Ukranian

Please select the appropriate language that you speak so we can arrange an interpreter for your assessment. It is likely that the interpreter will be present via video or telephone. 

Adaptations

We are committed to ensuring our service is easy to access and inclusive. If you require any adaptations or adjustments, please let us know on the referral form in the adaptations section. 

What happens after I submit my referral?

It is ok if you are feeling a bit anxious, and not sure how it all works. Please see our what to expect page for more information that will hopefully put you at ease. 

Within a few days after you submit your referral, you will get a text message. It will have a link to book your first phone appointment. You can choose the most convenient day and time. You can also rearrange your appointment via this link upto 24 hours before. 

If we think another service fits your needs better, we will send you a text message. It will ask you to check with your GP alternative forms of support.
